Nurse Practitioner Ebony
Ebony Fortier, MSN, APRN, FNP-C, PMHNP-BC, partners with patients on their journey toward improved physical and mental well-being. As a Board-Certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, she combines evidence-based care with a whole-person approach that considers the connection between the mind, body, lifestyle, and overall wellness.
With nearly a decade of clinical experience and advanced training in functional and integrative medicine, Ebony is passionate about helping patients better understand the factors that may be influencing their health. She believes meaningful healing begins with listening, education, and personalized care that addresses the unique needs of each individual.
Her mission is to empower patients with the knowledge, tools, and support needed to make informed decisions about their health. Through a collaborative approach, Ebony helps patients create sustainable strategies that promote resilience, wellness, and long-term quality of life.

With over 13 years of experience as a Registered Nurse and Family Nurse Practitioner, Ebony brings a compassionate, whole-person approach to patient care. As a Board-Certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, she supports patients experiencing digestive concerns, mental health challenges, inflammation, skin conditions, and other wellness concerns.
Ebony believes health is influenced by many interconnected factors, including nutrition, lifestyle, physical health, stress, and emotional well-being. She combines evidence-based medicine with functional and integrative principles to help patients better understand their health and create personalized plans for lasting wellness.
All visits are conducted virtually through phone or HIPAA-compliant video, allowing patients to receive convenient, individualized care from the comfort of their homes.
Growing up in California, Ebony always knew she wanted to help others improve their health and well-being. Her own health journey led her to discover functional medicine after struggling with persistent skin concerns and realizing the importance of addressing lifestyle, nutrition, gut health, and overall wellness.
After earning her Master of Science in Nursing from South University, Ebony served patients in pediatrics, chronic disease management, functional medicine, and behavioral health.
As a Board-Certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, she is passionate about helping patients understand the connection between physical and mental health through a whole-person approach to care.
Ebony lives in Oklahoma with her three daughters and is honored to serve patients throughout Oklahoma.
